If you’ve begun to notice the signs of aging, it’s likely that you’ve noticed loose skin around the eyes. Loose skin around the eyes is largely due to genetic causes.  Over time, the eyelids are one of the first places to sag and lose firmness.  This can make you look tired, sad or unhappy. In some cases, loose skin can also affect vision. Loose skin around the eye is called dermatochalasis, and upper and lower lid blepharoplasty procedures are effective plastic surgery solutions.

What is Dermatochalasis?

Dermatochalasis refers to loose skin around the eyelids.  This can cause a “baggy” appearance to the lower eyelid and a “hooded” appearance to the upper eyelid.  In the upper eyelid, the loose skin can hang down into the visual field, causing less light to reach the pupil.  This can make the vision poor at night or in other low light situations. Gynecomastia is a common condition that can be disturbing to many men.  Unfortunately many myths still persist today that cause men to be discouraged when seeking treatment for gynecomastia.

Myth: ‘Pseudogynecomastia, Man boobs, Male Breasts, Gynecomastia are mutually exclusive (you have one or the other)”

Dr. Azouz:  Most men have a combination of fatty tissue and gynecomastia.  It is not uncommon for men to have massive weight loss and have true gynecomastia (pure breast tissue).  Similarly men who are at their ideal weight may have significant fatty tissue in their chest.  Most men will have a combination of gynecomastia (breast tissue) and fatty tissue.

Myth: ‘Nipple Size is independent of gynecomastia’

Dr. Azouz: Enlarged male Nipples are most often associated with underlying gynecomastia.  Patients undergoing gynecomastia surgery can often expect their nipple areola to reduce by half to one third of the size.

Myth:  ‘You will have Drains with Gynecomastia Surgery’

Dr. Azouz: We do not use drains in gynecomastia surgery.  This allows a more comfortable recovery without additional scarring from drain sites.

Dr. Azouz: Older men can have wonderful results with gynecomastia surgery.  We operate on men in their 70s and 80s routinely and have many older patients who are very happy after male breast reduction surgery.

Myth: ’Teenagers and pre teens are not candidates for gynecomastia surgery’.

Dr. Azouz: Young adults and teens can have wonderful results with gynecomastia surgery.  We operate on boys as young as 10 and 12.  There are patients who have severe gynecomastia even before they are teenagers.  Gynecomastia surgery can be performed in even growing boys.

Myth: ’Male breast reduction is very painful’.

Dr. Azouz: Many of our patients do not take any pain pills after male breast reduction surgery.  Some patients only take Tylenol and advil.  It is rare for a patient to ever need a refill of their pain prescription after surgery.

Recovery time for removal of breast implants is somewhat dependent on if a total capsulectomy is performed or not.  In general recovery from removal of implants is rapid with patients returning to work a few days after surgery.  It is not recommended to engage in heavy exercise activity for four to six weeks after surgery.

The cost of breast implant explantation is dependent on the type of implant and the need for a total capsulectomy.  Insurance coverage is possible for women with advanced capsules.  Many insurance companies deep breast implants to be cosmetic and therefore coverage is dependent on the insurance carrier and policy.

Breast feeding is possible after breast augmentation, breast implant removal, breast reduction, and  breast lift.  Patients who have undergone a mastectomy for breast cancer will be unable to breast feed as the breast glands, ducts and nipples may have been removed.

What is gynecomastia?

Although breasts do not develop in males like they do for females, most men are born with a small amount of breast tissue. A male’s body makes the testosterone hormone which guides his sexual growth and development with puberty. However, males also produce the female sexual growth hormone, estrogen. Endocrine studies have demonstrated a hormonal imbalance with an increase in estrogen, leading to a swelling and hypertrophy of male breast tissue. Almost 50 percent of adolescent boys develop gynecomastia to some degree and some will develop permanent gynecomastia presenting as a female appearing breast or as puffy nipples.  Some men will also present with lipodystrophy, which is an abnormal distribution of fat overlying the pectoralis muscles. Many men have a combination of both abnormal fat distribution and gynecomastia (breast tissue) appearing to be female breasts.

What is gynecomastia?

 

Gynecomastia is a condition that can first present in puberty or after infancy, as the young man is encountering many different changes in his body. Two to Three Days Post Liposuction Surgery

One to Three Weeks Post Liposuction Surgery

One to two weeks after liposuction patients will avoid exercise and most patients do not have any reportable pain.  Many liposuction patients can go back to desk work or light duties at this point.  Full compression is utilized during this time period as advised by Dr. Azouz.  All stitches will usually be removed from the small liposuction incisions between 2 to 3 weeks after surgery.

Three to Six Weeks Post Liposuction Surgery

At three to four weeks after liposuction surgery, results become much more dramatic.  Bruising and swelling usually improve dramatically in the first three to four weeks.  Most liposuction patients can return back to light physical activity after the third week including heavy walking, elliptical, light yoga and light aerobics.

Six Weeks Post Liposuction Surgery

Liposuction patients are usually released to full activity after six weeks. Most patients can additionally return to all physical activities or exercise routines. This includes basketball, hockey, football, heavy yoga, trampoline, skydiving and scuba diving.

Myth: ‘Nipple Size is independent of gynecomastia’

Dr. Azouz: Enlarged male Nipples are most often associated with underlying gynecomastia.  Patients undergoing gynecomastia surgery can often expect their nipple areola to reduce by half to one third of the size.

Myth:  ‘You will have Drains with Gynecomastia Surgery’

Dr. Azouz: We do not use drains in gynecomastia surgery.  This allows a more comfortable recovery without additional scarring from drain sites.

If you’ve begun to notice the signs of aging, it’s likely that you’ve noticed loose skin around the eyes. What is Dermatochalasis?

Dermatochalasis refers to loose skin around the eyelids.  This can cause a “baggy” appearance to the lower eyelid and a “hooded” appearance to the upper eyelid.  In the upper eyelid, the loose skin can hang down into the visual field, causing less light to reach the pupil.  This can make the vision poor at night or in other low light situations. What is diastasis recti?

A diastasis of the rectus abdominus muscles refers to the laxity or weakening of the major muscles in the abdomen.  These muscles separate and cause a protuberant or “bulging” abdomen. Some patients feel that their abdomen ”looks pregnant” because of the weakening of their rectus diastasis.

Which Tummy Tuck Procedure is Right for Me?

There are a few different modalities of the Tummy Tuck surgery. These are the full abdominoplasty, mini-abdominoplasty, and no-drain abdominoplasty. Different modalities will work for different patients based on their specific needs. 

Full Abdominoplasty

The full Tummy Tuck involves a horizontal incision below the lower abdomen and an incision around the navel. This procedure removes excess skin while also repairing the muscles and pulling them to a tighter position. This is commonly used for patients who have issues from pregnancy or major weight fluctuations such as patients who have had Bariatric Surgery (Post Bariatric). 

Mini Abdominoplasty

With the mini Tummy Tuck, only one incision smaller incision along the lower abdomen is needed. It pulls the skin to a tighter position. It is best used for minor cases of laxity. The best candidates for this procedure have most of their laxity below the navel.   

No-Drain Abdominoplasty

Dr.  David Azouz has been performing no drain Tummy Tucks for over 25 years. This technique is to reduce the amount of fluid that is built-up after surgery, by closing off the space where fluid would accumulate.  No drain (drainless) surgery avoids painful drains, avoids excess scarring and does not require the frequent monitoring and emptying of fluid.

What is the Tummy Tuck Recovery Like?

Recovery is different for each patient. Dr. Azouz will provide specific aftercare instructions that should be followed closely for optimal healing.   Dr. Azouz utilizes multiple techniques that minimize pain with a Tummy Tuck. This includes the method of anesthesia, local nerve blocks, no drain approach, cold therapy and more.  Swelling and bruising is normal but should subside within weeks after surgery. Dr. Azouz has patients avoid steam rooms and saunas for three months after surgery so as to swell. Most people are free to return to their normal routine within one to two weeks.   Heavy activity such as scuba diving, horseback riding, basketball, hockey, as well as heavy lifting is out for six weeks.

Can a Tummy Tuck Be Combined with Other Procedures?

The Tummy Tuck is often combined with other surgeries for a more comprehensive approach to addressing patient concerns. Dr. Azouz routinely uses liposuction with a Tummy Tuck to remove fatty tissue in and around the abdomen. Also, a Tummy Tuck is one of the main procedures done in a mommy makeover. A mommy makeover is a comprehensive surgery that uses procedures that address the abdomen, breasts, and fatty tissue on the body.  Patients will often have a breast lift, breast augmentation or labiaplasty concurrently with their Tummy Tuck.  

Do I need to lose weight before a Tummy Tuck?

Often Dr. Azouz does not necessarily recommend losing weight prior to a Tummy Tuck.  A Tummy Tuck can be a great way to improve exercise and diet. Dr. Azouz does recommend quitting smoking prior to a Tummy Tuck.

Does Dr. Azouz perform liposuction with a Tummy Tuck?

Can my belly button be saved with a Tummy Tuck?

Where are the scars located in a Tummy Tuck?

A Tummy Tuck usually involves a scar hidden within the underwear line and a scar around the belly button.  The belly button is a scar from the loss of the umbilical cord at birth. This means that belly button scars are usually camouflaged once they fade.

When will I see the results of a Tummy Tuck?

How does a neck lift compare to non-surgical procedures?

Surgery

Dr. Azouz most commonly performs neck lift surgery with local anesthesia and sedation. Dr. Azouz uses this technique so you are comfortable while minimizing the risks associated with general anesthesia.  This also allows Dr. Azouz to monitor and protect the muscles of facial animation.  Patients can be deepened or lightened to their desired level of comfort with anesthesia.  The vast majority of patients do not remember their operation due to the specific medications that are utilized during cosmetic procedures.  Dr. Azouz works with anesthesiologists and certified nurses and anesthetists who are experts in delivering an optimal surgical experience.  Side effects such as pain and nausea are minimized by Dr. Azouz’s local and regional blocks, his specialized dressings and the long-lasting anesthesia drugs that are provided by the anesthesia provider.

Recovery

Dr. Azouz provides prescriptions for medications that will treat pain and nausea after a neck lift.  Patients do not usually describe significant pain after a neck lift and there are many patients who do not take any opioid pain pills after surgery.  Dr. Azouz personally follows his patients in the office after surgery with frequent appointments.  Dr. Azouz removes stitches at different appointments and will notify patients when they begin their skincare, sunscreen and makeup regimens.  Neck lift incisions are hidden behind the ears and under your chin.  Most patients are able to return to desk work a few days after surgery.  Dr. Azouz does not recommend any heavy activity such as scuba diving, sky diving, cross-fit, heavyweight training and marathon running for at least four to six weeks after surgery.

Frequently Asked Facelift Questions

How is a neck lift different than a facelift?

A Neck lift only addresses the lower portion of the face including the neck and jowls.  Young patients who have loose skin and fatty tissue under the chin are more commonly candidates for a neck lift.  Older patients who would like the benefit of midface rejuvenation are better candidates for facelifts.  Dr. Azouz generally combines a neck lift with all facelift procedures.  Some patients simply require neck liposuction which has the additional benefit of skin tightening and minimal incisions.

How can you tell the difference between platysma bands and loose skin?

Platysma bands are actually strips of muscles of the neck which become prominent with aging.  This is due to a combination of soft tissue and skin laxity as well as decreased muscle tone.  Dr. Azouz will be able to diagnose and treat platysmal banding after an in-office history and physical examination.

Is Botox enough for neckbands or is a platysmaplasty better?

Neuromodulators such as botox and Dysport generally only will last for 2-4 months.  This means that patients will have to have their platysma bands treated with injections several times a year for as many years as the plastysma bands bother them.  With a neck lift or facelift Dr. Azouz treats platysma bands directly by a platymaplasty, direct platysma band excision and by resuspending the skin and soft tissues of the face.

What causes premature aging?

The overwhelmingly greatest cause of premature aging is genetics.  People often notice that they look more and more like their aging parents as they age.  There are modifiable causes of aging as well but these do not contribute to aging as much as genetics.  Modifiable causes of aging include sun exposure, smoking, diet, skincare, and stress.

How can I reduce premature aging and maintain the results of my neck lift or facelift?

  1. Sun protection is one of the best ways to prevent premature aging. Wearing hats, avoiding the sun at the hottest parts of the day and applying sunscreen frequently is critical.  New broad-spectrum sunscreens protect from multiple damaging rays including UVA/UVB, infrared and blue rays.
  2. Smoking causes significant damage to nearly all parts of the body. The skin and soft tissues of the face and neck are damaged and can not recover as well in smokers.  Smoking can also cause discoloration of the teeth and gums.  Quitting smoking can not only lead to a longer life but also to the ability to better rebuild collagen and heal.
  3. Eating healthy is another way to better prevent early aging. A healthy diet rich in fruits and vegetables provides the skin with a healthy glow, better texture, tone and can even contribute to better skin elasticity.  Alcohol, fried, salty and sugary foods do not provide the face with the nutrients to build healthy and youthful collagen.
  4. Daily skincare programs with retinA, hydroquinone, vitamin c and moisturizers can strengthen, smoothe and tighten the skin. Azouz customizes skincare regimens to his patients after performing a skin exam and history.
  5. Daily stress is difficult to avoid but patients who provide self-care with exercise, meditation, familial support, and yoga can better distress to lower their stress hormones such as cortisol and adrenaline. Even very young patients in their 20s and 30s can become aged looking after experiencing periods of significant stress.

What do I need to know before I have liposuction?

How long is the postoperative period after liposuction?

Dr. Azouz performs liposuction as a day surgery.  Duration of recovery does depend on the amount of fat removed, history of the patient, and duration of surgery.  In general Dr. Azouz performs liposuction under sedation so that a patient does not remember the surgery and is sleeping.  Sedation allows for the patient to be “asleep” but does not involve placing a breathing tube past the vocal cords.  This results in a much more rapid recovery the day of surgery and is associated with a higher safety profile.Dr. Azouz does recommend that you are accompanied postoperatively for the first 12 hours, by a relative or friend.

How is Breast Implant Removal Performed?

Breast implant removal surgery is done to remove implants from a previous augmentation procedure. It corrects breast implant-related issues such as capsular contracture, scar tissue, or rupture, as well as any cosmetic concerns.  Liposuction techniques can also play a role to remove bra fat or to remove silicone collections. Breast implant removal and breast implant revision are usually done under intravenous sedation without the need for surgical drains.

The stitches used are generally absorbable and often will not need to be manually removed. So … do I HAVE to change my breast implants?

Not necessarily, some patients who are not having issues with their implants do not have to change their implants.  It is recommended to follow up with a plastic surgeon to assess the implants and breasts regularly for breast screening and examination.  In some patients, a lift can be done around the implants to remove extra skin without removing or altering the implants.
